المستخلص:
تهدف الدراسة إلى إبراز أهمية موضوع الأمراض السرطانية في محافظة القادسية لقلة الدراسات الجغرافية عنها ولأنها تعد من
المواضيع المهمة التي تناولتها الدراسات والبحوث على كافة المستويات العالمية
والدولية والمحلية. ولضرورة معرفة العوامل والمسببات المرضية ذات التأثير المباشر
في حدوثها والمتمثلة بالعوامل البيئية الجغرافية (الطبيعية والبشرية) والعوامل
الذاتية والوراثية والتي ساهمت بدورها في تزايد معدلات ونسب الإصابة بالأمراض
السرطانية في محافظة القادسية في السنوات الأخيرة من الدراسة.
توصلت الدراسة إلى نتائج علمية أهمها هو أن التوزيع الجغرافي المكاني
والزماني والتغيرات السنوية للمصابين بالأمراض السرطانية في محافظة القادسية قد
اتخذت نمطاً متفاوتاً ومتذبذباً من مدة زمنية لأخرى، فالتباين المكاني والزماني
لمعدلات الإصابة بالأمراض السرطانية الأكثر شيوعاً وانتشاراً في منطقة الدراسة نتج عن التفاوت بحالات الإصابة خلال
المدد الزمنية الثلاث ( 1990 - 2000) و (2001 - 2010) و ( 2011 -
2019) فقد سجل مر سرطان الثدي أعلى نسب الإصابة خلال تلك المدد ، ثم تبعتها
الإصابات بمرض سرطان الرئة، ومرض سرطان الدم والرحم مقاربة لها، ثم تأتي الإصابات
بمرض المثانة وسرطان الغدد اللمفاوية.
واستنتج من ذلك تزايد حالات الإصابة بداية من الأعوام التي أعقبت العدوان الأمريكي على العراق عام 1991،
وأخذت بالتزايد التدريجي بعد حرب الخليج الثانية عام 2003، والتي شهدت زيادة
متذبذبة تارة ومنخفضة تارة أخرى بسبب الأخطاء الواردة في سجلات المراجعين شهرياً
وسنوياً في منطقة الدراسة، وارتفاع نسبة الملوثات البيئية التي خلفتها الاسلحة
المحرمة دولياً من قذائف اليورانيوم المنضب، وما تركت من مخلفات بيئية ملوثة شملت
جميع مكونات الحياة في عموم العراق بشكل عام، ومنطقة الدراسة على وجه الخصوص. وما
خلفت من واقع مؤلم مرير وتدني بالخدمات الصحية والطبية وعلى كافة المستويات حيث
عانت منها منطقة الدراسة منذ أعوام عديدة ومازالت تعاني من افتقارها لأبسط مقومات
العناية الصحية وانعدام الكفاءة الطبية في جميع مستشفيات المحافظة والمراكز الصحية
التخصصية. حيث قدمت الدراسة وصفاً جغرافياً وتحليلاً كمياً أعطى خصائص مهمة للكشف
عن وجود ارتباط اتجاهات المرض السرطاني بالخصائص الديموغرافية والمتمثلة (التركيب
النوعي والعمري والوظيفي) ومدى تباين الأهمية النسبية لكل منها والعوامل المؤثرة
في انتشار المرض بين فئات عمرية دون غيرها.
أظهرت الدراسة أن مرض السرطان يأتي بالمرتبة الثانية من حيث أعداد
الوفيات في منطقة الدراسة بعد أمراض القلب والشرايين. وأن مرض السرطان مشكلة صحية
في غاية الخطورة والمسبب الرئيسي للوفيات نتيجة تراكم عوامل بيئية جغرافية ولا
سيما عامل التلوث البيئي الذي لعب دوراً مهما في نشوء المرض وارتفاع حالات الإصابة
به والوفاة خاصة بعد سنوات الحروب التي توالت على العراق منذ عام 1991، إذ شهدت
محافظة القادسية تبايناً مكانياً بين الانخفاض والارتفاع بنسب أعداد وفياتها
بالأمراض السرطانية بين وحداتها الإدارية لمدة الدراسة والبالغ مجموعها الكلي
(1233 حالة وفاة)، إذ بلغ المعدل بحسب النوع نحو (1.2 بالألف) للذكور يقابل (0.5
بالألف) للإناث وهذا التباين في معدل الوفيات النوعي يعود لأسباب متعددة منها
طبيعية وأخرى بشرية وأن قضاء الديوانية جاء بمعدل وفيات للذكور بلغ (1.0 بالألف)
وهو أقل من معدل الوفيات للإناث البالغ (0.5 بالألف) مقارنة بأقضية المحافظة
الأخرى، ويعزى السبب في ذلك إلى أن مركز المحافظة أعداد سكانه أكثر والخدمات
الصحية المقدمة أفضل من الاقضية والنواحي التي سجلت أعداد وفيات أعلى من ذلك. أما
بالنسبة للتباين المكاني لمعدلات الوفيات في المحافظة فأشارت الدراسة إلى ظهور
أربع مراتب هي الأولى جاء فيها قضاء الشامية بنواحيه الإدارية بأعداد وفيات خام
بلغ المعدل العام لها (1.2 بالألف) وهو كمعدل أكثر من باقي أقضية المحافظة الأخرى
ويعزى السبب إلى أنه يعد من المناطق الريفية المعروفة بتدهور الخدمات الصحية
والتغذية في وانعدام مقومات النظام الصحي في وافتقاره لمراكز الفحص المبكر المخصصة
للأورام السرطانية ، المرتبة الثانية احتلها قضاء الحمزة بمعدل وفيات خام بلغ (1.0
بالألف) مجموع كل من ناحيتي السدير والشافعية. والثالثة اقتصرت على مركز قضاء
الديوانية بمعدل وفيات خام بلغ (0.8 بالألف) والرابعة وهي الأخيرة والتي احتلها
قضاء عفك بمعدل وفيات خام بلغ (0.7 بالألف) وهو الأقل على مستوى الأقضية الأخرى
بناحيتي (نفر وسومر) لعدم الاكتراث في تسجيل بعض حالات الوفاة.
جاءت نتائج الدراسة الميدانية لمعرفة الاختلافات والفروقات في جودة
الحياة بأبعادها المختلفة، والتعرف على الجوانب الاقتصادية والنفسية والاجتماعية
والمهنية وتباين بعد كل منها على المصابين، واستنتج من ذلك بأن الواعي الصحي في
المحافظة متردي ويعاني من عجز كبير في عدد مراكز الرعاية الصحية الرئيسية والأولية
ومؤسساتها الصحية ووجود تباين مكاني غير متكافئ بين وحداتها الإدارية بمناطقها
الحضرية والريفية. مما يحتاج إلى إجراء دراسات تقويمية دورية سنوية لتقييم الواقع
الصحي لمرضى السرطان في المحافظة. فضلا عن إعداد خطط توسعية للبنية التحتية
للرعاية الصحية في عموم البلاد، ومن أولوياتها تطوير المستشفيات ومراكز الأورام
السرطانية لأجل تحسين واقع الخدمات الصحية بشكل شامل، والعمل على بناء مستشفى خاص
للأمراض السرطانية في المةحفظة مجهز بكافة الوسائل والمعدات الضرورية للمعالجة على
وفق الأسس الاستراتيجية للرعاية الصحية لمكافحة حدوث الإصابة بالمرض والوقاية منه
التي أوضحتها الدراسة.

Abstract:
The study aims to highlight the importance of the issue of cancerous
diseases in Al-Qadisiyah governorate due to the lack of geographical
studies about it and because it is one of the important topics addressed
by studies and research at all global, international and local levels.
And for the need to know the factors and pathogens that have a direct
impact on their occurrence, which are represented by the geographical
environmental factors (natural and human), intrinsic and genetic
factors, which in turn contributed to the increase in the rates and
rates of cancerous diseases in Al-Qadisiyah Governorate in the last
years of the study.
The study reached scientific results, the most important of which is
that the geographical, spatio-temporal distribution and annual changes
of patients with cancerous diseases in Al-Qadisiyah governorate have
taken a varying and fluctuating pattern from one time period to another.
The three time periods Breast cancer recorded the highest )(1990-2000),
(2001-2010) and (2011-2019) rates of infection during those periods,
respectively, followed by lung cancer cases at rates and the rates of
leukemia were similar to them, as well as the incidence of uterine
cancer, and then the cases of bladder disease come at lower, It was
concluded from this .followed by lymphoma, which are few, and recorded
that the cases of infection increased starting from the years that
followed the American aggression on Iraq in 1991, and took a gradual
increase after the second Gulf War in 2003, which witnessed a
fluctuating increase at times and low at other times due to the errors
contained in the auditors’ records monthly and annually in the study
area, and the high rate The environmental pollutants left by the
internationally prohibited weapons of depleted uranium shells, and the
contaminated environmental residues left by all components of life
throughout Iraq in general, and the study area in particular. And the
bitter painful reality that it left behind, and the decline in health
and medical services at all levels, the study area has suffered for many
years and is still suffering from its lack of the most basic elements
of health care and the lack of medical competence in all the
governorate’s hospitals and specialized health centers. Where the study
provided a geographical description and a quantitative analysis that
gave important characteristics to reveal the existence of a link between
the trends of cancerous disease with the demographic characteristics
represented by (specific, age and functional composition) and the extent
of the variation in the relative importance of each of them and the
factors affecting the spread of the disease among age groups without
others.
The study showed that cancer ranks second in terms of the number of
deaths in the study area after cardiovascular diseases. And that cancer
is a very serious health problem and the main cause of death as a result
of the accumulation of geographic environmental factors, especially the
environmental pollution factor, which played an important role in the
emergence of the disease and the high incidence of it and death,
especially after the years of wars that continued on Iraq since 1991,
Al-Qadisiyah governorate witnessed a spatial discrepancy between the
decrease and increase in the rates of the number of deaths from cancer
among its administrative units for the duration of the study, totaling
1,233 deaths. The qualitative reason is due to various reasons,
including natural and human, and that Al-Diwaniyah district came with a
death rate for males that reached (1.0 per thousand), which is lower
than the mortality rate for females of (0.5 per thousand) compared to
other districts of the governorate, and the reason for this is due to
the fact that the governorate center has more population and the health
services provided Better than the districts and districts that recorded a
higher number of deaths. As for the spatial variation of mortality
rates in the governorate, the study indicated the emergence of four
ranks, the first of which came in the district of Al-Shamiya in its
administrative areas with the numbers of raw deaths, the general rate of
which reached (1.2 per thousand), which is as a rate more than the rest
of the other districts of the governorate. The reason is attributed to
the fact that it is one of the well-known rural areas With the
deterioration of health and nutrition services in it and the lack of the
components of the health system in it and its lack of early examination
centers dedicated to cancerous tumors, and the second place was
occupied by the district of Hamza, with a crude death rate of (1.0 per
thousand) the sum of each of the two districts of Al-Sudair and
Al-Shafi’i. The third was confined to the center of Al-Diwaniyah
district, with a crude death rate of (0.8 per thousand), and the fourth,
which is the last, which was occupied by Afak district, with a crude
death rate of (0.7 per thousand), which is the lowest in the other
districts in the Nafar and Sumer sub-districts, for not caring about the
registration of some deaths.
The results of the field study came to know the differences and
differences in the quality of life in its various dimensions, and to
identify the economic, psychological, social and professional aspects,
and the variation of each one of them on the injured. Unequal spatial
disparity between its administrative units in its urban and rural areas.
This requires conducting annual periodic evaluation studies to assess
the health status of cancer patients in the governorate. As well as
preparing expansion plans for the infrastructure of health care
throughout the country, and among its priorities is the development of
hospitals and cancer tumor centers in order to comprehensively improve
the reality of health services, and work to build a special hospital for
oncological diseases in the governorate equipped with all the necessary
means and equipment for treatment in accordance with the strategic
foundations of health care to combat The incidence and prevention of
disease explained by the study.
